PHYSICAL EDUCATION CONTRACT

 

This is a contract between _______________________ and Mr. Johnson (instructor) for a PE course. 

By signing this contract I am acknowledging that my grade will be reduced and/or I will be removed

from the class when I do not follow these rules.

 

1. I will make every effort to follow the assigned workout/activity each day.

 

2. I will demonstrate and practice good conduct while in the weight room or gym.  I understand

there will be no horseplay or goofing off while under the contract.  Severe or continual horseplay

 could result in a failing grade or removal from class.

 

3. I realize that a combination of 15 absences (non-school related), tardies, or nonsuits can result in failing the class.  Unexcused absences cannot be made up.

 

4. I will refrain from using profanity, put-downs, or gestures.

 

5. I will dress down in uniform for class each day.  If I’m sick or have an injury I will dress down and participate to the best of my ability.  If I am too sick to participate I should NOT be at school.  Long-term injuries may result in study hall.  I understand that non-suits cannot be made up. 

 

6. I will follow the proper procedures for entering and exiting the gym/locker room.  I will wait in the gym area until the bell rings or I will receive a Referral.

 

7. I am responsible for completing any assignments, tests, or physical testing that I miss.

 

8. I must have permission to leave the gym or weight room at all times.  This includes not going out into the gym hallway or bathrooms without permission.

 

9. I understand that when the whistle blows I must stop what I am doing, including talking, to listen to directions.

 

10. I understand that whining usually results in a tougher workout or loss of student choice.
